Foundayo vs Rybelsus: Two Daily GLP-1 Pills, Different Molecules, Different Jobs

Both are once-daily GLP-1 tablets. One is a small molecule approved for weight management; the other is a peptide approved for type 2 diabetes, and in the U.S. it now goes by a new name. Here is how they actually differ.

The short answer

Foundayo and Rybelsus get compared because they look alike on a pharmacy shelf: a tablet, once a day, that activates the GLP-1 receptor. In practice they solve different problems for different patients.

Key takeaway

Foundayo (orforglipron, Eli Lilly) is FDA-approved for chronic weight management in adults with obesity, or overweight plus a weight-related condition. You can take it any time of day, with or without food. Rybelsus (oral semaglutide, Novo Nordisk) is approved to improve blood sugar control in adults with type 2 diabetes and to lower major cardiovascular risk in high-risk adults with type 2 diabetes. It is not approved for weight management, and it comes with a strict empty-stomach morning routine.

One more thing to know before you read further: in the U.S., Novo Nordisk is retiring the Rybelsus brand name. The FDA approved "Ozempic" as the proprietary name for Novo's oral semaglutide tablets on February 4, 2026, and the reformulated "Ozempic pill" launched in the U.S. on May 4, 2026. The current FDA label covers both Rybelsus tablets (3, 7 and 14 mg) and Ozempic tablets (1.5, 4 and 9 mg). Same molecule and same indications. The strengths are not interchangeable milligram for milligram. Where this guide says "Rybelsus," read it as "oral semaglutide for type 2 diabetes, under either name."

Two molecules, two engineering problems

Semaglutide is a peptide, a chain of amino acids modeled on the body's own GLP-1 hormone. Peptides are normally broken down in the stomach, which is why most semaglutide is injected. Rybelsus gets around this with an absorption enhancer and a very particular way of taking it. That is where the fasting rule comes from (more on that below).

Orforglipron is not a peptide. Lilly describes it as a small-molecule, non-peptide GLP-1 receptor agonist that is chemically synthesized, not built from amino acids. It binds to and activates the human GLP-1 receptor. Lilly's medical information page reports that in a phase 1 study, about 79% of a single 0.8 mg dose reached circulation. Because it does not depend on an empty stomach to be absorbed, Foundayo's label allows dosing with or without food.

So both pills land on the same receptor. They get there by very different routes, and the route drives the daily experience.

What each is approved to treat

Foundayo

Weight management

FDA-approved April 1, 2026. Used with a reduced-calorie diet and more physical activity:

  • Adults with obesity, or
  • Adults with overweight plus at least one weight-related condition
  • Not for use with another GLP-1 receptor agonist
Rybelsus / Ozempic pill

Type 2 diabetes

Label revised 2025–2026. Used alongside diet and exercise:

  • Improves glycemic control in adults with type 2 diabetes
  • Reduces the risk of major cardiovascular events in high-risk adults with type 2 diabetes
  • No weight-management indication

Two practical consequences follow. First, Foundayo is not FDA-approved for type 2 diabetes. In a February 26, 2026 release, Lilly said a U.S. type 2 diabetes submission was planned for later in 2026. We could not confirm a submission or an FDA decision as of this writing. Second, prescribing Rybelsus mainly for weight loss in someone without diabetes is off-label. Insurers (and Medicare) usually tie coverage to the approved indication.

If weight management is the goal and you don't have diabetes, the oral semaglutide product that is FDA-approved for it is a different one: Wegovy tablets. We compare those two semaglutide pills in Wegovy pill vs. Rybelsus.

Dosing rules: fasting vs. none

This is the difference patients notice from day one. Everything below comes from the current FDA labels. Your prescriber's instructions take priority.

Rybelsus and Ozempic tablets

  • One tablet, once daily, in the morning on an empty stomach.
  • Take it with no more than 4 ounces of plain water. No other liquids.
  • Wait at least 30 minutes before eating, drinking or taking any other oral medicine.
  • Swallow whole. Do not split, crush or chew.
  • The first strength (3 mg Rybelsus or 1.5 mg Ozempic tablet) is a 30-day starter dose. The label notes it is not effective for blood sugar control. The dose steps up after 30 days and may increase again after day 60 if more control is needed.
  • Missed dose: skip it and take the next dose the following day.

Foundayo

  • One tablet, once daily, at any time of day, with or without food. Swallow whole.
  • Starts at 0.8 mg. After at least 30 days it goes to 2.5 mg, then after at least 30 more days to 5.5 mg. After at least 30 days at each step, your prescriber may increase it to 9, 14.5 or 17.2 mg (the maximum).
  • Missed dose: take it as soon as possible, but never two in one day. If you miss 7 or more days in a row, the label says escalation restarts at a lower dose, so call your prescriber.
Why it matters

The 30-minute window affects more than breakfast. If you take a thyroid pill, blood pressure medicine or anything else by mouth in the morning, all of it has to wait. That is a real reason some people prefer Foundayo. Don't switch on your own, though. They are approved for different conditions, and each has its own interaction list.

More detail on getting the semaglutide routine right is in our guide on how to take oral semaglutide correctly.

What the trials show

Head-to-head: ACHIEVE-3

The most direct comparison is Lilly's ACHIEVE-3 trial (results announced February 26, 2026, and published in The Lancet). It ran 52 weeks and was randomized but open-label, so participants knew which drug they were taking. It enrolled 1,698 adults with type 2 diabetes who weren't well controlled on metformin. They were assigned to orforglipron 12 or 36 mg, or oral semaglutide 7 or 14 mg. Average starting A1C was 8.3%.

ACHIEVE-3 results at 52 weeks (efficacy estimand, per Lilly)
ArmA1C changeWeight changeStopped for side effects
Oral semaglutide 7 mg−1.1 pts−3.7%4.5%
Oral semaglutide 14 mg−1.4 pts−5.3%4.9%
Orforglipron 12 mg−1.9 pts−6.7%8.7%
Orforglipron 36 mg−2.2 pts−9.2%9.7%

Source: Eli Lilly press release, Feb. 26, 2026. The trial's orforglipron doses were capsules. Lilly says the 12 mg and 36 mg capsules correspond to the 9 mg and 17.2 mg Foundayo tablets sold in the U.S., and that capsules and tablets aren't interchangeable mg for mg. The oral semaglutide doses match the original Rybelsus strengths.

Two honest caveats. Orforglipron lowered A1C and weight more, and more people on it stopped because of side effects. And because Foundayo isn't FDA-approved for type 2 diabetes, these results describe a use that isn't on its U.S. label yet.

Each drug's own approval evidence

Foundayo was approved on two 72-week placebo-controlled trials. In adults with obesity or overweight without diabetes (3,127 people), average weight change at the 17.2 mg dose was −11.1%, versus −2.1% on placebo. In adults with type 2 diabetes and a BMI of 27 or higher (1,613 people), it was −9.6% versus −2.5%. These are the label's intention-to-treat figures. Lilly also cites a higher 12.4% figure for people who stayed on treatment.

Rybelsus trials measured blood sugar. In the label's monotherapy trial, A1C fell 1.2 points on 7 mg and 1.4 points on 14 mg, versus 0.3 points on placebo, at 26 weeks. Rybelsus also has a cardiovascular outcomes result. In the SOUL trial (14 mg, median follow-up about four years), major cardiovascular events occurred in 12.0% of participants versus 13.8% on placebo. That result supports its heart-risk indication in type 2 diabetes. Foundayo has no completed cardiovascular outcomes trial on its label.

Side effects and interactions

Both drugs mostly cause gastrointestinal side effects, which are most common while doses are going up. Both carry a boxed warning about thyroid C-cell tumors. They are contraindicated if you or a family member has had medullary thyroid carcinoma, or if you have MEN 2.

  • Foundayo (pooled weight trials): nausea in 26–35% of people depending on dose, versus 10% on placebo; constipation 20–27% vs. 9%; diarrhea 21–25% vs. 11%; vomiting 13–24% vs. 4%. About 8% stopped because of side effects, versus 3% on placebo.
  • Rybelsus (placebo-controlled diabetes trials): nausea in 11% at 7 mg and 20% at 14 mg, versus 6% on placebo; abdominal pain 10–11% vs. 4%; diarrhea 9–10% vs. 4%.

Those rates come from different trial populations, so don't compare them directly. ACHIEVE-3 is the only like-for-like comparison, and the drop-out figures are in the table above.

Interactions worth raising with your prescriber

Foundayo: the label says it may reduce how well birth control pills are absorbed. It advises switching to a non-oral method, or adding a barrier method, for 30 days after starting and after each dose increase. Strong CYP3A4 inhibitors cap the dose at 9 mg, and strong CYP3A4 inducers should be avoided. Simvastatin should not exceed 20 mg a day.

Rybelsus: it raised total thyroxine exposure in a levothyroxine study. The label suggests closer monitoring of oral drugs that need careful dosing.

Both: if you also take insulin or a sulfonylurea, the risk of low blood sugar goes up, and your prescriber may lower those doses.

  • Commercial insurance: both companies advertise savings cards that can bring eligible copays down to as little as $25. Both exclude people with government coverage such as Medicare or Medicaid.
  • Medicare: Part D can cover oral semaglutide for type 2 diabetes, subject to each plan's formulary. Foundayo has no diabetes indication. For weight management, it is one of the drugs in the temporary Medicare GLP-1 Bridge, which charges a $50 copay for eligible enrollees from July 1, 2026, through December 31, 2027. People with type 2 diabetes are excluded from the Bridge. Coverage basics are in our 2026 insurance coverage guide.

Who each one fits

Foundayo may fit if
  • Your goal is weight management and you meet the label's criteria
  • A fixed 30-minute fasting window doesn't work with your mornings or other medicines
  • You can handle a longer titration (at least 30 days per step)
  • You're prepared to adjust birth control around dose changes, if that applies
Rybelsus / Ozempic pill may fit if
  • You have type 2 diabetes and want an oral GLP-1 for blood sugar
  • Proven cardiovascular-risk reduction in type 2 diabetes matters to you
  • Your plan covers it as a diabetes drug
  • You can reliably follow the empty-stomach morning routine

If neither fits cleanly, for example weight management without diabetes but with established heart disease, the Wegovy pill has its own label and its own evidence. See Wegovy pill vs. Foundayo and our comparison tool.

Frequently asked questions

Is Foundayo the same as Rybelsus?

No. Foundayo contains orforglipron, a small-molecule GLP-1 receptor agonist from Eli Lilly approved for chronic weight management. Rybelsus contains semaglutide, a peptide from Novo Nordisk approved for type 2 diabetes and cardiovascular risk reduction in type 2 diabetes. Both are once-daily tablets that activate the GLP-1 receptor.

Is Rybelsus being renamed Ozempic?

In the U.S., yes. The FDA approved Ozempic as the proprietary name for Novo Nordisk's oral semaglutide on February 4, 2026, and the reformulated Ozempic pill (1.5, 4 and 9 mg) launched May 4, 2026. Its strengths are not interchangeable milligram for milligram with Rybelsus 3, 7 and 14 mg, so any switch should go through your prescriber.

Do I need to fast before taking Foundayo?

No. Foundayo's label says to take it once daily at any time of day, with or without food. Rybelsus and Ozempic tablets must be taken on an empty stomach in the morning with up to 4 ounces of water, with a 30-minute wait before eating, drinking or other oral medicines.

Which pill lowered A1C more in a head-to-head trial?

In Lilly's 52-week, open-label ACHIEVE-3 trial in adults with type 2 diabetes on metformin, orforglipron lowered A1C by 1.9 to 2.2 points versus 1.1 to 1.4 points with oral semaglutide 7 or 14 mg. More people on orforglipron stopped because of side effects. Foundayo is not FDA-approved for type 2 diabetes as of October 2026.

Can I take Rybelsus for weight loss?

Rybelsus is not FDA-approved for weight management, so prescribing it mainly for weight loss would be off-label, and coverage is usually tied to the approved diabetes indication. The oral semaglutide product approved for weight management is Wegovy tablets. Talk to your prescriber about which product matches your diagnosis.
