The FDA has approved several oral medications for chronic weight management. The newest — the Wegovy pill (oral semaglutide 25mg) — entered the market in January 2026 and has quickly become the highest-profile option. But it is not the only one, and it is not always the best fit for every patient.

Wegovy pill (oral semaglutide)

Mechanism: GLP-1 receptor agonist. Average weight loss: 13–17% of body weight. Dose: 25mg daily. Cost: $149–$299/month. Key advantage: highest average weight loss of any oral medication. Key disadvantage: strict dosing requirements (empty stomach, 30-minute wait), GI side effects. Best for: patients who want maximum weight loss in pill form and can comply with the dosing protocol.

Contrave (naltrexone/bupropion)

Mechanism: opioid antagonist + antidepressant combination targeting appetite centers in the brain. Average weight loss: 5–8% of body weight. Cost: $100–$300/month. Key advantage: may help patients with binge eating or food addiction patterns. Key disadvantage: contraindicated with opioids, less effective than GLP-1s for pure weight loss. Best for: patients with co-occurring depression or binge eating, or those who cannot tolerate GLP-1 medications.

Qsymia (phentermine/topiramate)

Mechanism: stimulant + anticonvulsant combination. Average weight loss: 8–10% of body weight. Cost: $100–$200/month. Key advantage: longest FDA-approved track record, solid efficacy data. Key disadvantage: contains a controlled substance (phentermine), requires REMS program enrollment, pregnancy category X. Best for: patients who respond well to stimulant-based appetite suppression and are not planning pregnancy.

Orlistat (Xenical / OTC Alli)

Mechanism: lipase inhibitor — blocks fat absorption in the gut. Average weight loss: 3–5% of body weight. Cost: $30–$60/month (OTC Alli). Key advantage: available over the counter, no prescription needed for the lower dose. Key disadvantage: GI side effects (oily stools, urgency) are unpleasant and directly tied to fat intake. Best for: patients who need a low-cost, low-barrier starting point and can tolerate the GI effects.

How to decide

The Wegovy pill is the most effective oral weight loss medication by a significant margin. If your primary criterion is weight loss magnitude and you can manage the dosing protocol, it is the clear first choice. But medical history, contraindications, cost, and individual response all factor in. Contrave may be better for patients with mood or behavioral eating components. Qsymia may be appropriate for patients who have tried GLP-1s without adequate response. Orlistat is a reasonable starting point for patients exploring pharmacotherapy for the first time.
