Oral Peptide Absorption: The Pharmacokinetics Behind Oral GLP-1 Dosing

Oral GLP-1 dosing instructions can seem arbitrary — fasting requirements, small water volumes, specific waiting periods. They aren't arbitrary. Here's the pharmacokinetics that explain why the instructions matter.

The Digestion Problem

The gastrointestinal tract is designed to break peptides down into amino acids for absorption. Enzymes in stomach acid and intestinal contents degrade peptide chains rapidly. For most peptide drugs — insulin being a famous example — oral administration is largely infeasible because the drug is destroyed before it can be absorbed.

Semaglutide, being a peptide, faces this challenge. Solutions have required either avoiding the oral route entirely (injectable formulations) or engineering the oral formulation to briefly modify gastric conditions to enable absorption.

How Absorption Enhancers Work

SNAC — the absorption enhancer used in Rybelsus and Wegovy pill — works by transiently affecting the local pH and membrane permeability in the stomach where the tablet dissolves. This creates a brief window during which semaglutide can be absorbed across the gastric mucosa before being degraded.

The window is narrow and specific. Food in the stomach disrupts the local conditions. Water beyond a small volume dilutes the effect. Other oral medications taken too soon interfere. Hence the dosing instructions: empty stomach, small water volume, waiting period.

Compounded Oral Formulations

Compounded oral GLP-1 formulations use various approaches to enable absorption. Sublingual troches attempt to absorb through the oral mucosa, bypassing the stomach entirely. ODTs may or may not include absorption enhancers depending on the compounding pharmacy's formulation. Oral drops similarly vary in their approach.

The variability across compounding pharmacies means patient experience can differ meaningfully between formulations from different sources. This isn't necessarily a quality problem — it's the nature of individualized compounded product.

The Timing Instructions

For brand-name products, follow the dosing instructions precisely — they've been calibrated in trials to produce the expected absorption. For compounded products, follow the specific instructions from your prescriber and pharmacy — they may differ from brand-name protocols depending on formulation.

What Absorption Variability Means

Individual variability in oral GLP-1 absorption is greater than variability in injectable absorption. Some patients absorb more efficiently, some less. This is one reason titration and individual response monitoring matter for oral GLP-1 therapy — patient response is a better guide to dose optimization than trial-average expectations.
