Why Oral GLP-1s Cost More Per Milligram: The Bioavailability Math

Oral GLP-1 formulations require higher milligram amounts of active ingredient than injectable to produce comparable clinical effect. This is why per-milligram pricing on oral compounded product looks higher than injectable. Here's the underlying math.

The Bioavailability Concept

Bioavailability describes what fraction of an administered drug reaches the bloodstream in unchanged form. Injectable subcutaneous administration has near-100% bioavailability — the entire dose reaches circulation. Oral administration of peptides, by contrast, has bioavailability typically well below 5%, sometimes below 1%, because most of the peptide is broken down in the digestive tract before absorption.

What This Means for Dosing

To achieve the same blood levels of semaglutide from oral administration as from a 1 mg injectable dose, an oral formulation needs to contain many times more active ingredient — the exact ratio depending on the formulation's absorption efficiency. Compounded oral formulations typically use significantly higher milligram amounts than injectable equivalents. This is by design and clinically appropriate for the format.

Why Per-Milligram Pricing Looks Different

If you compare the raw milligrams of active ingredient in a monthly oral supply to a monthly injectable supply, the oral supply contains meaningfully more milligrams. Yet the retail price for a monthly supply is comparable or higher for oral. Per-milligram calculation makes oral look expensive — but the metric isn't apples-to-apples because you need more milligrams to get the same effect.

The clinically relevant comparison is cost per month of effective treatment, not cost per milligram. On that basis, oral and injectable land in similar ranges.

The Absorption Enhancer Approach

Brand-name oral semaglutide (Rybelsus, Wegovy pill) uses SNAC (sodium N-[8-(2-hydroxybenzoyl) amino] caprylate) as an absorption enhancer to improve bioavailability. This still doesn't reach injectable-level absorption, but improves oral absorption enough to enable clinically meaningful blood levels at manageable oral dose sizes.

The Small Molecule Alternative

Orforglipron and other small molecule GLP-1 agonists (still in clinical development) address the bioavailability problem structurally rather than through absorption enhancers. Small molecules have inherently better oral absorption than peptides. If these agents deliver on Phase 3 promise, they could change the per-milligram cost dynamics of the oral GLP-1 category meaningfully.
